Clinicians who want to participate in the EAP Network with FEI must first submit a completed Clinician Profile to FEI Behavioral Health. Once the Clinician Profile is received and processed, a CAQH standard welcome packet will be sent to the clinician or designated credentialing administrator if the clinician is not already registered with CAQH. The welcome packet contains all the information and directions needed to complete the initial credentialing application.
Clinicians who are already registered with CAQH, and who have authorized FEI to receive their data, may only receive an email to re-attest to their data. If their attestation is current, they will receive no communication and their credentialing will be initiated. Upon request, a clinician has the right to inquire about the status of his/her credentialing application at any time during the credentialing process. FEI Behavioral Health will review the application and contact the clinician if any additional information is needed before the application is approved or denied.
If you would like to add clinicians to your organization, please have the clinician complete a Clinician Application along with the CAQH Provider Data Form and submit to the Network Operations Department. Email or fax: 414-359-1973.

As a provider for FEI Behavioral Health, we ask that you review the Provider Handbook (contact FEI for a copy). Make it available to all FEI-approved clinicians and make copies for all branch offices.
Once you have reviewed the Provider Handbook, we ask that you provide training to all FEI-approved clinicians to ensure that they have read it and understand FEI’s standards and procedures for handling employee assistance program (EAP) cases.
If you have any questions, please do not hesitate to contact FEI Behavioral Health's Network Operations Department for more information, (414) 359-1055 followed by option 2. We would be more than happy to review the manual with you or provide any additional information.
In order to process payment, claim forms must be completed and returned to FEI's claim department within 90 days of the first session. Also, note that all sessions must be preauthorized by FEI Behavioral Health.
We appreciate working with you and look forward to continuing to work with you in the future. Your feedback and suggestions are important to us.
If you have any questions, please do not hesitate to contact us.
Thank you for your cooperation.
