Clinician Applications

All clinicians of FEI Behavioral Health are required to complete an application and submit it to FEI Behavioral Health's Provider Relations Department.

Affiliate/Provider Forms

As an FEI affiliate, we ask that you review the Affiliate Handbook (contact FEI for a copy), make it available to all FEI approved clinicians and make copies for all branch offices. Once you have reviewed the handbook, we ask that you provide training to all FEI approved clinicians to ensure that they have read it and understand FEI’s standards and procedures for handling EAP cases.

If you would like to add clinician to your organization, please have the clinician complete a Clinician Application and send it to FEI.

In order to process payment, claim forms must be completed and returned to FEI's claim department within 30 days of each session. Also, note that all sessions must be preauthorized by FEI Behavioral Health.
